Insurance & Financing
Making treatment fit your budget
A great smile and a good night's sleep shouldn't be out of reach. We'll check your benefits and help you find a plan that works for your family.

Braces & Invisalign: dental insurance
Many dental plans include an orthodontic benefit, usually a lifetime maximum. For orthodontics, in-network and out-of-network benefits are generally the same, so you can use your benefit with us either way. We accept assignment and bill your insurance for you.
A typical orthodontic benefit covers about 50% of treatment, up to a lifetime maximum of around $1,500 to $2,000 for each covered person. Every plan is different, so we'll check yours and build your payment plan around what your insurance will pay.
Plans we commonly work with include:
- MetLife
- Delta Dental
- Cigna PPO
- Aetna PPO / DMO
- Guardian
- and many more
Sleep apnea: medical insurance
Sleep apnea treatment is billed to your medical insurance, not your dental plan. Unlike many offices that offer oral appliances, we're contracted as an in-network provider with many medical insurance companies, which usually means lower out-of-pocket costs for you.
Before you start, we'll check your benefits and explain your copay, coinsurance and deductible, so you know what to expect.
In network
- AARP Medicare Advantage
- Aetna
- Banner
- Blue Cross Blue Shield (BCBS)
- Medicare
- Optum
- TriWest
- TRICARE For Life
- UnitedHealthcare (UHC)
- UHC Choice Plus HSA
- Oscar
- UMR
Out of network, but we can still treat you with a referral
We're working on getting in network with every insurance company. Until then, we can still see patients with these plans with an out-of-network referral:
- Allwell by Wellcare
- Alignment Health
- Banner Aetna
- Centene
- Cigna (covers the oral appliance, but not office visits)
- HealthSpring / Cigna
Plans we don't accept
- AHCCCS plans, including Mercy Care and Banner University Family Care (BUFC)
- UHC Community Plan / UHC Dual Complete
- Ambetter
What you'll pay
Your cost for sleep apnea treatment depends on your medical plan's deductible, copays and coinsurance. We generally ask for payment when services are done. If you have a high deductible, our billing team can work out a short-term payment plan with you.
No insurance? Self-pay is welcome
We see self-pay (cash) sleep apnea patients at a discounted rate, and payment plans are available.
Sleep apnea patients: pay your bill online
Our billing team uses a secure online patient portal. Log in anytime to make a payment and see your current and past balances, what your insurance paid, and any adjustments on your account.
Go to the Patient Billing Portal
Can't log in? Contact our billing office at (623) 233-4585 or [email protected], and we'll send you an invitation to set up your portal account. Billing e-fax: (833) 330-2854.
Payment plans for braces & Invisalign
We offer our own 0% interest in-house payment plans, so there's no outside finance company involved. Those companies charge fees to both patients and the practice. Financing your treatment ourselves keeps those fees out of it.
- Braces: a smaller down payment, then monthly payments over the course of treatment. Many families put about 10% down and spread the rest over 18 to 24 months, depending on treatment time, at 0% interest.
- Invisalign: a larger down payment, since it covers the lab cost of making your custom aligners, then monthly payments.
- Pay in full: prefer to pay it all up front? Ask about our paid-in-full discount.
Your exact down payment and monthly amount depend on your treatment and what your insurance covers. We'll go over all of it at your free consultation, with no pressure.
Ways to pay
We accept Visa, Mastercard, Discover and American Express. HSA and FSA cards are welcome too; they run just like a regular debit or credit card.
